Ohio requires drivers under 18 to complete 50 hours of supervised driving — including 10 hours at night — before the road test. What the BMV 5791 requires
The affidavit certifies 50 hours of behind-the-wheel practice with a licensed adult, at least 10 of them at night. An eligible adult signs before a notary, and the attached Supervised Driving Log records each drive's date, the supervising driver and their Ohio license number, and the start and end time.
The state's app — and what it collects
Ohio also offers a free app — built by a private vendor with the state's Traffic Safety Office — that logs the hours and generates the BMV 5791. Using it is optional; the paper affidavit is still accepted. If you're weighing it, read its privacy policy first. As of July 2026, the app's store listings and privacy policy describe collecting precise GPS location (including in the background) and driving telematics such as speed and braking, and the policy permits sharing personal information with third parties; Ohio officials have described the driving data as managed by the app's provider.
