How to fill out Missouri Form 4901
Missouri Form 4901 is a convenience record for the 40 supervised hours required before an intermediate license, including 10 hours at night. It records individual drive sessions and supports the qualified adult who verifies the hours at the license office.

Use a separate row for each drive
Enter the date for the actual session. If two drives happened on the same date, keep separate rows when their start and end times or trainers differ.

Write the start, end, and total time
Record the clock time when the drive began and ended, then enter the session’s total duration. Check that the total agrees with the interval instead of estimating it later.

Identify the trainer
Enter the supervising trainer’s name for every drive. Confirm before counting the session that the adult is qualified under Missouri’s current instruction-permit rules.

Track night hours outside the row total
Form 4901 has no separate night column even though at least 10 of the 40 hours must occur at night. Maintain an accurate night subtotal alongside the form so the qualified adult can verify that minimum.

Track both hours and elapsed permit time
Reach at least 40 supervised hours, including 10 at night, but also account for the instruction permit’s 182-day holding period. Finishing the practice early does not shorten that waiting period.

Use the log for in-person verification
A qualified adult accompanies the teen to the license office and verifies the completed hours. Form 4901 is kept as the supporting convenience log rather than mailed in as a separate application.
Two drives on one Saturday — two rows, not one
Form 4901 is per drive, not per date, because each row carries its own start time, end time, and trainer. A Saturday with a morning lesson and an evening lesson produces two separate rows.

Nothing on either row marks the evening drive as night driving — Form 4901 has no night column. Those 45 minutes still count toward Missouri’s 10-hour night minimum, so the family needs a night subtotal kept somewhere the form does not provide.
What goes wrong at the license office
- Combining multiple drives into one date row and losing the correct start, end, or trainer details.
- Recording only a duration even though Form 4901 also provides start-time and end-time fields.
- Assuming the form’s Grand Total proves the 10 night hours even though the rows do not identify night driving separately.
- Assuming 40 completed hours replace the separate 182-day instruction-permit hold.
What fills each 4901 row — and what it cannot
Missouri’s export is per drive rather than per date, matching the form’s one-outing-per-row shape:

The 182-day permit hold is not on the form either. Reaching 40 hours quickly does not shorten it, so it is worth watching the calendar and the hour total as two separate things.
